What NEMT brokers require

Commercial auto liability

$1,000,000 combined single limit minimum. Broker minimums usually run above your state's Medicaid floor — the broker minimum governs.

General liability

$1,000,000 per occurrence / $2,000,000 aggregate is the common baseline. Some hospital-based contracts want more.

Workers' compensation

Statutory limits — brokers commonly require it even where it's technically optional for your state.

SAM endorsement

Sexual abuse and molestation coverage, required or strongly preferred, and it has to actually appear on the certificate.

Licensed across 8 states: Louisiana, Texas, Georgia, Alabama, Arkansas, Mississippi, Oklahoma, and Tennessee.

What gets credentialing delayed

  • Auto liability below the broker's $1,000,000 minimum

  • No SAM endorsement listed

  • Broker's legal name wrong or missing in the additional insured field

  • No 30-day advance cancellation notice endorsement

  • Additional insured shown on the certificate but never actually endorsed onto the policy
